The Opportunity:
The Clinical Nurse Liaison works closely with neurologists and clinic staff to help patients receive timely, coordinated care. The nurse supports people living with multiple sclerosis and other neurological conditions, including Parkinson’s disease, cognitive disorders and other complex conditions. This is a hands-on role combining patient care, treatment access, clinical coordination and practical support for day-to-day clinic operations.
The successful candidate will be approachable, organized and comfortable using technology. They will take initiative, identify problems early and work with physicians, patients, pharmacies, insurers and support programs to find solutions. The nurse will be an integrated member of two neurology practices, while remaining part of the Sentrex Clinic Solutions team.
A Day in the Life (What you will do here):
Patient care and clinical support
- Support patient visits, review relevant medical records, collect clinical information and document care accurately in the electronic medical record (EMR).
- Conduct cognitive screening and standardized questionnaires, including depression and anxiety screening, as directed by the physician.
- Administer prescribed injections and vaccinations within nursing scope, monitor patients as required, and maintain safe vaccine storage, inventory and fridge documentation.
- Provide patient education on neurological conditions, prescribed therapies, self-injection where applicable, and the importance of staying on treatment.
- Help coordinate laboratory work, imaging, genetic testing and other investigations, including required requisitions and follow-up.
Treatment access and reimbursement
- Prepare and submit complete public drug coverage applications (including Ontario Exceptional Access Program), private insurance authorizations, renewals and patient support program forms.
- Monitor application status, follow up on missing information, address denials and work with the physician and support programs to explore alternatives such as compassionate access when appropriate.
- Coordinate therapy starts, changes and renewals, including required testing, infusion readiness and communication with pharmacies or infusion providers.
- Look for ways to simplify and improve forms and treatment access workflows, so physicians can review and sign accurate, complete documentation.
Preventive care and patient centered support
- Support clinic wellness checks, including vaccination status, cardiovascular risk factors (such as blood pressure, lipids and Framingham risk), and appropriate cancer screening follow-up.
- Review medication information and relevant bloodwork, flag concerns and coordinate follow-up with the physician.
- Help connect patients to mental health, cognitive behavioural therapy and addiction services when needed, recognizing that these needs can affect treatment adherence.
Clinic coordination, systems and improvement
- Use and help maintain clinic systems, including Accuro EMR, electronic forms, e-prescribing, hospital report and laboratory result interfaces, and government drug coverage portals.
- Work closely with reception and administrative staff to keep patient records, forms, referrals, appointments and follow-up tasks organized.
- Troubleshoot everyday workflow and technology issues, identify gaps and help introduce practical improvements in collaboration with the physicians and Sentrex team.
- Communicate clearly and respectfully with patients, caregivers, physicians, pharmacies, insurers and other care providers.
- Support both neurology practices on an agreed weekly schedule and participate in Sentrex training, team communication and quality improvement activities.
Professional practice and safety
- Work within applicable nursing scope, physician orders, College of Nurses of Ontario standards and Sentrex policies.
- Maintain patient privacy, accurate documentation, safe medication handling and required adverse event reporting.
- Keep required clinical skills, CPR and professional education current; seek guidance when a clinical issue is outside the role’s scope.
What you need to ensure you are set up for success:
- Registered Nurse (RN) or Registered Practical Nurse (RPN) with current registration in good standing with the College of Nurses of Ontario.
- Two to four years of clinical nursing experience preferred; neurology, outpatient clinic or chronic disease experience is an asset.
- Comfortable giving intramuscular and subcutaneous injections and managing vaccines; willing to complete role-specific training.
- Strong computer skills and confidence learning EMRs, online portals and electronic forms; Accuro experience is an asset, not a requirement.
- Friendly, patient-focused communication skills, including the ability to put patients at ease during assessments and follow-up calls.
- Strong attention to detail, initiative, organization and independent problem-solving; able to follow an issue through to resolution.
- Experience with drug coverage applications, patient support programs or specialty medications is an asset.
- Current CPR and First Aid certification, valid driver’s licence and ability to travel between the Barrie and Orillia clinics.
What makes you a great fit for this team:
